This course provides an in-depth overview of the importance of creating and maintaining accurate records and reports. Delegates will explore a range of different recording requirements including care planning, incident reports and safeguarding reports. Delegates will develop skills and understanding in how to complete clear, accurate and factual records that contribute to overall excellence in recording practice.

Learning objectives:
• Describe recording requirements in adult health and social care
• Identify responsibilities and accountability in relation to record keeping, including when to escalate information for further action
• State the importance of clear written English in effective record keeping
• Explore barriers and enablers for good practice in record keeping and report writing
• Explain the difference between fact and opinion in recording and report writing
• Identify relevant information for different types or records and reports including care planning, incident reports and safeguarding reports
• Identify the key features of well written records and reports
• Describe how to maintain confidentiality in relation to record keeping
• Outline the audit process and the importance of good record keeping in relation to this
