Dementia Best Practice – Discharge to Assess – Cohort 03 D2A

This session is for managers, team leaders, activity leads and seniors, and focuses on best practice in dementia care to avoid crisis in Discharge to Assess placements.

Prior to attending this course, delegates are encouraged to complete the pre-learning module Introduction to Prevalence and Types of Dementia

• Person centred care linking into DTA Specification and connected lives
• Behaviours of concerns
• Positive Behaviour Support Approaches
• ABC Charts
• Supporting wellbeing – activity life history environment and approach.
• Spotting escalation and how to try to deescalate
• Risk and positive risk taking
• Therapeutic observation
• How and when to seek support
• Impact of hospital admissions
• Diagnostic overshadowing and utilising the symptom checklist
