Below is a letter from Adult Care Services Residential Commissioning, HCPA are continually working on your behalf and would like to add to this that behind the scenes please rest assured that HCC the CCG’s and HCPA are working hard with the hospital Trusts to try to achieve more timely planning of discharges so there is not a backlog at holiday periods and this is not felt as if it is a burden on care providers. We completely understand this is a system challenge. This is a big issue and may take more time for us come up with some solutions, we will certainly be piloting some ideas over the winter period to see what works and hope you will work with us on this.

As you will be aware, the bank holiday weekend often means extra pressure across health and social care services. Our Hospital Teams are extremely busy in trying to arrange care for people who are fit to leave hospital and I’m sure you will be facing similar pressures in ensuring high quality care continues to be provided over the weekend.
The Council is extremely thankful to the help and assistance providers give during these busy times and as on previous occasions again ask for extra efforts in helping the vulnerable people of Hertfordshire during this busy time. In particular, we ask that:
Re-admissions back into your home happen quickly. Admission decisions are made and communicated to our placement and discharge teams ideally within two hours of referral. If possible, admissions can be made up to 8pm and across the weekend. Use the Impartial Assessors referrals and assessments to save time of having to visit the hospital. Update Herts Care Search daily to ensure that our placement and discharge teams can identify vacancies and send referrals effectively.
