One day course at HCPA Campus
This course, for staff working in Homecare & Supported Living settings, will support delegates to understand and discuss the relevance of Advance Care Planning (ACP) to their setting, and understand key principles and tools essential for effective planning. Delegates will learn to recognise and utilise important documents that support ACP and decision-making processes. Additionally, delegates will be equipped to identify, facilitate, and document crucial discussions that underpin ACP, enhancing their ability to support individuals in making informed decisions about their future care.

• Describe and discuss relevance of Advance Care Planning
• Understand key advance planning principles and tools
• Recognise and use key documents to support Advance Care Planning, and decision making
• Understand and consider how to recognise, facilitate and document important discussions that form the basis of ACP.
